Businesses make calls internally using a PBX (Private Branch Exchange). The PBX also connects to the public switched telephone network (PSTN) via trunk lines. Because they incorporate telephones, fax machines, modems, and more, the general term "extension" is used to refer to any end point on the branch.

PBXs are differentiated from "key systems" in that users of key systems manually select their own outgoing lines, while PBXs select the outgoing line automatically. Hybrid systems combine features of both.

Initially, the primary advantage of PBXs was cost savings on internal phone calls: handling the circuit switching locally reduced charges for local phone service. As PBXs gained popularity, they started offering services that were not available in the operator network, such as hunt groups, call forwarding, and extension dialing. In the 1960s a simulated PBX known as Centrex provided similar features from the central telephone exchange.

Voice over IP (VoIP) is just one of the thousands of services that require an internet connection.

One of the biggest issues being contested is the ability of internet service providers to favor their own content or limit access to content based on type.  For example, Comcast, one of the leading internet service providers also provides internet phone service or VoIP, without a truely nuetral internet, Comcast would be able to block other VoIP Providers such as Vonage.

Allowing internet service providers to limit consumers access to certain types of content, or favor their own brands would have a chilling effect on our entire economy.

NEW YORK — Voice over IP phone service provider Vonage Holdings Corp. has agreed to pay $3 million to 32 states to settle an investigation into some of its business practices.

In a filing with the Securities Exchange Commission, Vonage said it also agreed to provide refunds to affected customers.

Idaho Attorney General Lawrence Wasden said his office received complaints from consumers who said they found it difficult to cancel their service with Vonage amid pressure from the company to keep their accounts.

In Texas officials said that Vonage also failed to clearly tell potential customers that they needed high-speed Internet service to use Vonage, which offers cheaper calls by sending voice data over the Internet.  Officials said those unable to use the service had to pay cancellation and other fees.
